Usually the best car rental company from the US and Canada is 'Sixt'. Reservations can be made via Dollar rent-a-car 1-800-800-4000. Keep in mind that not all car rental companies outside of the Czech Republic permit driving the car to the Czech Republic, so if you fly elsewhere, make sure that the car rental company will let you do this (usually only certain models and for small extra fee).

If you fly directly to Prague you can rent a car from local rental company, usually cheaper than elsewhere, but choices of models might be limited to primarily local Skoda cars, you might want to check Czech Business Directory for some contacts.
